Myelofibrosis (MF) is a kind of myeloproliferative tumour with clinical manifestations of splenomegaly and anaemia. In Traditional Chinese Medicine, MF is classified under the categories of xulao (deficiency due to overstrain), jiju (accumulation-gathering) and suiku (marrow withering). Our team believes that the core pathogenesis of MF is blood deficiency, blood stasis, and is characterised by a mixture of deficiency and excess, and that as a chronic consumptive disease, blood deficiency and blood stasis are always present throughout its progression, so the treatment should focus on this core pathogenesis and take nourishing and activating the blood as the basic treatment, and evil excess pattern here is mainly characterized by the binding of phlegm and blood stasis, so it is supplemented with heat-removing and detoxicating herbs, in order to dispel the heat and toxicity and eliminate the evils of the toxins. Clinically, blood deficiency and blood stasis are the main manifestations, and Taohong Siwu Decoction (Peach Kernel and Carthamus Four Substances Decoction) is given as the basic formula to nourish and activate blood. If the symptoms of blood deficiency and blood stasis are less serious and the vital energy is still sufficient, Erchen Decoction (Two Matured Substances Decoction) is given to resolve phlegm and regulate qi, transport the spleen and harmonise the stomach to treat the evils of phlegm-stasis and toxin-stasis inter-conjugation. The application of the method of nourishing and activating blood, resolving phlegm and removing toxins in MF was also elaborated with clinical cases.
