摘要

非酒精性脂肪性肝病(NAFLD)是除外长期大量饮酒等因素所引起的一种代谢应激性肝脏损伤。中医学并无NAFLD这一病名，根据其常见临床表现将其归为“积证”“胁痛”“肝痞”“肝癖”等范畴。临床具体病因病机包括：肝郁脾虚、湿浊内停、湿热蕴结、痰瘀互结。角药是基于中药性味等原则，采用具有关联作用的3味药物，通过合理配伍提高临床疗效的中药配伍模式。临床可分别以调肝理脾、祛湿化浊、清解内热、化痰逐瘀之法为指导，灵活运用角药组方。柴胡、白芍、枳实疏解肝郁，人参、白术、茯苓健脾益气，茯苓、猪苓、泽泻利水除湿，杏仁、白蔻仁、薏苡仁清利三焦，牡丹皮、栀子、黄芩清解内热，半夏、陈皮、茯苓健脾化痰，牡丹皮、桃仁、赤芍活血化瘀。

Abstract

Non-alcoholic fatty liver disease (NAFLD) is a type of metabolic stress-induced liver injury

excluding factors such as prolonged excessive alcohol consumption. In traditional Chinese medicine

NAFLD is not recognized as a distinct diagnosis. Instead

it is categorized based on common clinical manifestations such as "accumulation syndrome"

"hypochondriac pain"

"abdominal masses in the liver"

and "liver lumps" . Specific clinical etiological factors and pathogenesis include the patterns of liver constraint with spleen deficiency

dampness retention

accumulation of damp and heat

and binding of phlegm and blood stasis. The use of Triangular Herbs involves a combination of three herbal medicines based on their properties and flavors

aimed at enhancing clinical effectiveness through rational compatibility. Clinical approaches guided by the Triangular Herbs include regulating and harmonizing the liver and spleen

draining dampness and removing turbidity

clearing heat

and eliminating phlegm and expeling stasis. Key herbal trios include Chaihu (Radix Bupleuri)

Baishao (Radix Paeoniae Alba)

and Zhishi (Fructus Aurantii Immaturus) for alleviating liver constraint

Renshen (Radix et Rhizoma Ginseng)

Baizhu (Rhizoma Atractylodis Macrocephalae)

and Fuling (Poria) for strengthening the spleen and replenishing qi

Fuling

Zhuling (Polyporus)

and Zexie (Rhizoma Alismatis) for draining water and eliminating dampness

Xingren (Semen Armeniacae Amarum)

Baikouren (Fructus Amomi Rotundus)

and Yiyiren (Semen Coicis) for clearing and facilitating the three

jiao

Danpi (Cortex Moutan)

Zhizi (Fructus Gardeniae)

and Huangqin (Radix Scutellariae) for clearing internal heat

Banxia (Rhizoma Pinelliae)

Chenpi (Pericarpium Citri Reticulatae)

and Fuling for fortifying the spleen and removing phlegm

and Danpi

Taoren (Semen Persicae)

and Chishao (Radix Paeoniae Rubra) for activating blood circulation and resolving stasis.
