摘要

目的

系统评价针刺治疗紧张型头痛的疗效及安全性。

方法

检索PubMed、Cochrane Library、Embase、Web of Science、中国知网、万方、维普、中国生物医学文献数据库(CBM)，检索时限均从2012年1月至2021年12月，收集针刺治疗紧张型头痛的随机对照试验研究。采用Revman5.3和STATA17.0对数据结局指标总有效率和VAS评分进行Meta分析。

结果

共纳入13项研究，累计样本量918例，其中治疗组427例，对照组491例。META分析结果：西药组：总有效率

＝1.20，

＝4.24(

0.000 1)；VAS评分SMD＝0.64，

＝4.61(

0.000 01)，针刺效果均优于西药。传统/常规针刺组：总有效率

＝ 1.21，

＝3.62(

0.05)；VAS评分SMD＝0.33，

＝2.77(

0.05)，特殊针刺疗效均优于传统/常规针刺。Egger检验法：总有效率：西药组

0.05，表示无发表偏倚；传统/常规针刺组

0.05，表示有发表偏倚；VAS评分：西药组

0.05，表示无发表偏倚；传统/常规针刺组

0.05，表示无发表偏倚。

结论

针刺治疗紧张型头痛在疗效与安全性方面均优于西药，特殊针刺手法相较于传统/常规针刺有更好的疗效。

Abstract

Objective

To analyze and evaluate the efficacy and safety of acupuncture in the treatment of tension-type headache (TTH).

Methods

We retrieved literature on the randomized controlled trials of acupuncture in the treatment of tension-type headache in Pub Med

Cochrane Library

EMBASE

web of science

CNKI

Wanfang

VIP and CBM dating from January 2012 to December 2021. Then

a Meta analysis was performed on the outcome indicators of total effective rate and VAS score with Revman5.3 and STATA17.0.

Results

Under the guidance of the inclusion and exclusion criteria

13 studies were included

with a cumulative sample size of 918 cases

including 427 cases in the treatment groups and 491 cases in the control groups. After the meta analysis

the following result were obtained: Western medicine group: total effective rate:

=1.20

=4.24 (

0.000 1); VAS score: SMD=0.64

=4.61 (

0.000 01)

indicating that curative effect of acupuncture was better than that of Western medicine. In the traditional / conventional acupuncture group: total effective rate:

= 1.21

= 3.62 (

0.05); VAS score: SMD = 0.33

= 2.77 (

0.05)

suggesting that curative effect of specialized acupuncture was better than that of traditional / conventional acupuncture. Egger regression test: total effective rate: Western medicine group

indicating no publication bias; In the traditional / conventional acupuncture group

Conclusion

In the treatment of TTH

acupuncture is superior to Western medicine both in efficacy and safety. Meanwhile

specialized acupuncture has better curative effects than traditional / conventional acupuncture.
