摘要

肉芽肿性乳腺炎又称肉芽肿性小叶性乳腺炎，是一种以乳腺小叶为中心的非干酪样坏死、以肉芽肿为主要病理特征的慢性炎症性疾病。其病情复杂、易反复，对女性身心健康影响较大，尚无较好治疗方案。燕京外科流派名家在内治调节体质基础上，精研外治，有效缩短疾病周期、促进疾病痊愈。具体分期治疗包括：急性期以外敷药膏为主，常用药物为芙蓉膏、复方化毒膏及铁箍散软膏。脓肿期治疗多顺势而为，若机体正气强盛，使用中医箍围法；若脓肿范围较深，多采用穿刺抽脓、垫棉绑缚法。肿块期以散结消肿为主，予消化膏湿热敷治疗。破溃期根据疮面脓腐情况及肉芽水肿程度不同，采用不同纱条进行换药，如朱红膏(红纱条)、甘乳纱条、血余蛋黄油、甘草油等。恢复期使用黑布药膏外敷，并在必要时施乳头内陷矫正术。

Abstract

Granulomatous mastitis

also known as granulomatous lobular mastitis

is a chronic inflammatory disease with non-caseous necrosis centered on the lobule of the breast and granuloma as the main pathological feature. Granulomatous mastitis is complex and easy to recur

it has a great impact on women’s physical and mental health and currently there is no proven cure for it. On the basis of internal treatment and physical conditioning

the famous experts of the Yenching School of TCM Surgeries made dedicated efforts into the external treatment

effectively shortening the disease duration and promoting healing. The staged treatment can be described as follows: In the acute stage

ointment is the main medicine

and the commonly used ones are Furong Ointment

Fufang Huadu Ointment and Tiegusan Ointment. The treatment in the abscess stage depends primarily on the patient’s conditions. If the body is strong

the TCM encircling therapy is employed; if the abscess is found in a deeper location

the method of puncturing for draining pus or cotton pad drainage are mostly used. In the lump stage

the focus is placed on dissipating masses and dissolving masses

which is achieved through moist heat compress with Xiaohua Ointment. During the rupture period

based on how badly the wounds suppurate and the degree of granulation edema

different yarns shall be used as the dressing

such as Zhuhong Ointment

(red yarn strips)

Ganru yarn strips

charred hair yolk oil

licorice oil

etc. During the convalescent period

heibu ointment is applied externally and

if necessary

inversion of retracted nipples may also be needed.
