摘要:The therapy of resolving phlegm and relaxing bowels now has been a major therapy for treating syndrome of phlegm heat and bowel excess of stroke in the fields of Chinese medicine and integrated Western and Chinese medicine. The authors reviewed the history and origin of the therapy, and introduced the discovery process of the syndrome from the visual angle of spring up. The authors observed bowel qi obstruction at the beginning of stroke, pondered over the simultaneous emergence of bowel qi obstruction and phlegm heat syndrome and relationship between the syndrome and disease severity, and analyzed the occurrence of syndrome of phlegm heat and bowel excess, accumulation of brain collateral damaged by internal toxin, and multiple ways of concurrence of the syndrome. Furthermore, the key points of syndrome differentiation, identification of syndromes, formula design and medication, modification, decocting methods, detailed rules, and application range of formulas according to clinical condition, observation indications, prognosis and contraindications of the syndrome, and clinical application of therapy of resolving phlegm and relaxing bowels were stated systematically in the paper.

摘要:Gastric cancer is one of the most common malignant tumors in China and has the highest incidence and mortality, which is a serious threat to people’s health and life safety. Precancerous lesions of gastric cancer(PLGC) are kind of pathological changes being easily transformed into cancer. The paper expounded the cognition of TCM on PLGC from the angles of etiology, pathogenesis, classification, views of different schools, disease differentiation and treatment. The authors thought that PLGC are the syndrome of deficiency in the root and excess in the tip. The deficiency in the root shows as spleen-stomach qi deficiency and/or spleen-stomach yin deficiency, and excess in the tip shows as blood stasis in stomach collateral and toxin damaging stomach collateral. The syndrome differentiation and treatment divides PLGC into three to five types. Different TCM physicians have various opinions and select various medicinals for treating the disease according its specific pathological changes. The problems existing in the research on PLGC were analyzed and future studies were prospected in the paper.

摘要:Objective To observe the influence of therapy of tonifying qi, resolving stasis and removing toxicity on c-myc gene in rats with chronic atrophic gastritis complicating dysplasia. Methods Wistar rats (n=60) were randomly divided into modeling group (n=50) and blank group (n=10). After successful modeling the rest of rats (n=30) were randomly divided into model group, Weimeisu group and Chinese medicinal group (each n=10). The model group was give normal saline water, Weimeisu group was given Weimeisu turbid liquid and Chinese medicinal group was given the Chinese medicinals with actions of tonifying qi, resolving stasis and removing toxicity (once a day). After continuous treatment for 12 weeks, the methods of real-time fluorescence quantitative polymerase chain reaction (RT-PCR) and Western blot were used for detecting c-myc gene in rats of all groups. Results The expression of c-myc mRNA increased significantly in model group compared with blank group (P<0.01), decreased significantly in Chinese medicinal group compared with model group (P<0.01), and decreased in Chinese medicinal group compared with Weimeisu group (P<0.05), and increased in Weimeisu group compared with blank group (P<0.05). The difference in the comparison between Chinese medicinal group and blank group had no statistical significance (P>0.05), and that between Weimeisu group and model group had no statistical significance (P>0.05). The expression of c-myc protein was significantly higher in model group than that in blank group (P<0.01), lower significantly in Chinese medicinal group than that in model group (P<0.01), lower in Weimeisu group than that in model group (P<0.05) and higher in Weimeisu group than that in blank group (P<0.05). The difference in the comparison between Chinese medicinal group and blank group had no statistical significance (P>0.05), and that between Chinese medicinal group and Weimeisu group had no statistical significance (P>0.05). Conclusion The therapy of tonifying qi, resolving stasis and removing toxicity can reverse gastric mucosal dysplasia through inhibiting the expressions of c-myc mRNA and c-myc protein.

摘要:Objective To investigate the theory basis of treating precancerous lesions of gastric cancer(PLGC) of chronic atrophic gastritis (CAG) from theory of qi deficiency and blood stasis through literature sorting out and summing up, and observe the curative effect the therapy of supplementing qi and activating blood on PLGC of CAG (syndrome of qi deficiency and blood stasis). Methods The theory basis of treating PLGC of CAG from theory of qi deficiency and blood stasis was summed up through literature sorting out. The patients (n=60) were randomly divided into treatment group and control group (each n=30). Treatment group was given Yiqi Huoxue Fang orally, and control group was given Weifuchun Pian orally. The therapeutic course was 6 months. The integrals of TCM symptoms, curative effect and integrals of pathological indexes were observed. Results The theory basis of treating PLGC of CAG from theory of qi deficiency and blood stasis was sufficient. The integrals of stomach duct pain and gastric stuffiness decreased in two groups after treatment (P<0.01). The integrals of loss of appetite, lack of strength, gastric upset, ructus and sloppy stool decreased in treatment group after treatment (P<0.01). The difference in the integrals of TCM symptoms had statistical significance between two groups after treatment (P<0.05), and the curative effect on TCM syndromes was better in treatment group than that in control group (P<0.01). The integrals of gastic atrophy and intestinal metaplasia decreased in two groups after treatment (P<0.01). The difference in integrals of gastic atrophy had statistical significance between two groups after treatment (P<0.05) and the curative effect was better in treatment group than that in control group. Conclusion The treatment of PLGC of CAG from theory of qi deficiency and blood stasis has sufficient theory basis, and the therapy of supplementing qi and activating blood has good curative effect on PLGC of CAG .

摘要:Objective To observe the influence of Chinese medicinals with actions of fortifying the spleen, clearing heat, activating blood and resolving stasis on gastric mucosal histological changes in rat model of chronic atrophic gastritis (CAG) accompanied by atypical hyperplasia for discussing the effective mechanism of Chinese medicinals. Methods All 70 rats were randomly divided into normal group, model group, modeling blank group, prevention group with Chinese medicinals (CMP group), prevention group with Western medicinals (WMP group), treatment group with Chinese medicinals (CMT group) and treatment group with Western medicinals (WMT group). The animal model of chronic atrophic gastritis accompanied by atypical hyperplasia was copied by using chemical stimulus method, and changes of gastric mucosal mitochondrial membrane potential (MMP) were detected by using flow cytometer. Results MMP increased in model group compared with normal group (P<0.05), decreased in CMP group and WMP group compared with model group (P<0.05). MMP increased in CMP group and WMP group compared with normal group (P<0.05). MMP decreased in CMP group compared with WMP group (P<0.05). The curative effect was higher in CMP group than that in WMP group. MMP increased in modeling blank group compared with normal group (P<0.05), and decreased in CMT group and WMT group compared with modeling blank group (P<0.05). MMP increased in WMT group compared with normal group (P<0.05). There was no statistical significance between CMT group and normal group (P>0.05). MMP decreased in CMT group compared with WMT group (P<0.05). The curative effect was higher in CMT group than that in WMT group. Conclusion The Chinese medicinals with actions of fortifying the spleen, clearing heat, activating blood and resolving stasis can treat CAG accompanied by atypical hyperplasia through reducing gastric mucosal MMP.

摘要:Objective To analyze and sum up Professor TANG Xu-dong’s medication experience on chronic atrophic gastritis (CAG) by applying data mining. Methods The materials were collected from 113 patients with CAG (148 visit times) treated by Professor TANG Xu-dong. The extract and input of materials and data structuring were carried out, and information collection module was built by applying the system of diagnostic and treatment information collection from famous and senior TCM physicians. The frequency analysis was used for analyzing and summing up the medication laws of common formulas and medicinals for different syndrome types. The data mining software was used for analyzing association rules of the corresponding combination relations between symptoms and medicinals and between medicinals and medicinals, and summarizing the laws in medicinal modification and combination. Results The main syndrome types of CAG included syndrome of liver depression and qi stagnation, syndrome of dampness-heat in spleen and stomach, spleen-stomach weakness syndrome, syndrome of depression-heat in liver and stomach and syndrome of stomach yin deficiency. The syndrome of liver depression and qi stagnation was treated with Xiangsu Yin, Sini San and modified Dan shan Yin, syndrome of dampness-heat in spleen and stomach, with Huangqin Huashi Tang, Banxia Xiexin Tang or modified Huanglian Wendan Tang, spleen-stomach weakness syndrome, with Huangqi (Radix Astragali seu Hedysari) combining modified Xiangsha Liujunzi Tang, syndrome of depression-heat in liver and stomach, with Xiangsu Yin combining Jinlingzi San and modified Zuojin Wan, and syndrome of stomach yin deficiency, with Yiwei Tang or Yiguan Jian. In the aspect of medicinal combination, common ones for regulating qi included the combinations among Xiangsu Yin, Zisuzi (Fructus Perillae), Chaihu (Radix Bupleuri), Zhiqiao (Fructus Aurantii), Sharen (Fructus Amomi Villosi) and Banxia (Rhizoma Pinelliae), and main combinations included Zisugeng (Caulis Perillae)+Chenpi (Pericarpium Citri Reticulatae)+Xiangfu (Rhizoma Cyperi), Chaihu+Zhiqiao and Zisuzi+Zisugeng. The common medicinal combinations for activating blood and regulating qi included Chuanlianzi (Fructus Meliae Toosendan)+Yanhusuo (Rhizoma Corydalis), Zisuzi+Xiangfu, Danshen (Radix Salviae Miltiorrhizae)+Sharen and crude Puhuang (Pollen Typhae)+fried Wulingzhi (Faeces Trogopterorum). The common medicinal combinations for clearing heat and resolving dampness included the combinations among Huanglian (Rhizoma Coptidis), alum processed Banxia, Huangqin (Radix Scutellariae), Huashi (Talcum), Roudoukou (Semen Myristicae), Fuling (Poria,), fried Baizhu (Rhizoma Atractylodis Macrocephalae) and Chenpi, and main combinations included Huashi+Huangqin+alum processed Banxia, Fuling+alum processed Banxia, fried Baizhu+alum processed Banxia, Roudoukou+alum processed Banxia and Roudoukou+Huashi. The common medicinal combinations for fortifying spleen and replenishing qi included the combinations among Dangshen (Radix Codonopsis), fried Baizhu and Fuling. The common medicinal combinations for clearing liver and purging heat included the combination of Zuojin Wan, and among Longdancao (Radix Gentianae) and Wuzeigu (Os Sepiella seu Sepiae). Conclusion Professor TANG Xu-dong’s medication experience on CAG is objectively summed up through applying data mining, which will provide the data support for deep study and comprehension of Professor TANG Xu-dong’s academic thought.

摘要:Objective To analyze the types of TCM syndromes and clinical indexes in patients with aristolochic acid nephropathy (AAN) and provide the evidence for the standardization of AAN diagnosis. Methods The types of TCM syndromes were observed and clinical indexes were compared in AAN patients (n=33) selected as study objects. Results Involving deficiency of healthy qi and among 33 cases, there were 27 with syndrome of spleen-kidney yang deficiency (81.82%), and 6 without syndrome of spleen-kidney yang deficiency (18.18%). Involving excess of pathogenic qi and among 33 cases, there were 18 with dampness turbidity syndrome (54.55%), and 15 without dampness turbidity syndrome (45.45%). The level of 24-hour urine protein was higher in group of spleen-kidney yang deficiency than that in group of non-spleen-kidney yang deficiency (P<0.05), and there was no statistical significance in other clinical indexes between two groups (P>0.05). The level of serum total cholesterol (TC) and urine specific gravity were higher in dampness turbidity group than those in non-dampness turbidity group (P<0.05), and there was no statistical significance in other clinical indexes between two groups (P>0.05). Conclusion The TCM syndromes are showed as mixture of deficiency and excess in AAN patients with spleen-kidney yang deficiency as the root, and dampness turbidity and blood stasis as the tip. There is a correlation between clinical indexes and syndrome types.
摘要:Objective To observe the curative effect of therapy of clearing lung and resolving phlegm on idiopathic pulmonary (interstitial) fibrosis (IIP, type of phlegm-heat obstructing lung) complicating infection. Methods The patients (n=81) with IIP were divided into control group (n=27) and treatment group (n=54). Control group was treated with intravenous drip of moxifloxacin, Mucosolvin and oral Acetylcysteine Capsules, and treatment group was given the decoction of Chinese medicinal with actions of clearing lung and resolving phlegm besides the same Western medicinal as control group. The curative effect on TCM syndromes, changes of symptom integrals, 6-minute walk distance test (6MWD), PO2 changes in blood gas analysis, and total lung capacity (TLC), breathing capacity (VC) and DLCO in lung function were compared between two groups. Results In treatment group, there were 10 cases (18.5%) with obvious effects, 35 (64.8%) with effects, 9 (16.7%) without effects, and the total effective rate was 83.3%. In control group, there were 3 cases (11.1%), 14 with (51.9%) effects, 10 (37.0%) without effects, and the total effective rate was 63.0%. U=-2.197, P=0.037 and P<0.05 after Ridit analysis. The scores of cough, plasma, fever, chest distress and pain, dry stool, upset, shortness of breath, yellow phlegm and moist rales were all decreased in two groups than those before treatment (P<0.05), which were more significant in treatment group (P<0.05). 6MWD was increased in two groups than that before treatment (P<0.01), which was more significant in treatment group (P<0.001). In blood gas analysis, PO2 increased in two groups than that before treatment (P<0.01), which was more significant in treatment group (P<0.01). In lung function DLCO increased in two groups than that before treatment (P<0.01), which was more significant in treatment group (P<0.001). Conclusion The therapy of clearing lung and resolving phlegm has a good curative effect on IIP (type of phlegm-heat obstructing lung).

摘要:Objective To discuss the curative effect of Yandi Chuxue Fang combining iontophoresis of Chinese medicinals on diabetic retinopathy. Methods The patients with diabetic retinopathy (n=80) were randomly divided into treatment group and control group (each n=40), and all treated with Yandi Chuxue Fang. The treatment group was given iontophoresis of Chinese medicinals besides of Yandi Chuxue Fang. The curative effect was observed in two groups. Results Of 40 cases in treatment group, there were 13 with obvious effect (32.5%), 20 with effect (50.0%), 7 without effect (17.5%) and 0 worsened, and the total effective rate was 82.5%. Of 40 cases in control group, there were 7 with obvious effect (17.5%), 18 with effect (45.0%), 15 without effect (37.5%) and 0 worsened, and the effective rate was 62.5%. The comparison in the curative effect between two groups showed as follows: Z=-2.152, P=0.031 and P<0.05 after rank-sum test. Conclusion Yandi Chuxue Fang combining iontophoresis of Chinese medicinals has better curative effect than simply oral Chinese medicinals on diabetic retinopathy.

摘要:Objective To observe the curative effects of incision therapy and comprehensive treatment on chronic throat impediment of type of phlegm coagulation and blood stasis. Methods The patients (n=100) were randomly divided into incision group (n=50) treated with incision therapy, and comprehensive group (n=50) treated with aerosol inhalation of Chinese medicinals, acupoint application and multifunction microwave irradiation. The curative effects were compared between two groups. Results There were 4 cases cured (8%), 24 with obvious effect (48%), 19 with effect (38%) and 3 without effect (6%), and the total effective rate was 94% in incision group. There were 5 cases cured (10%), 28 with obvious effect (56%), 15 with effect (30%) and 2 without effect (4%), and the total effective rate was 96%. The difference in the comparison of curative effect had statistical significance between two groups (P<0.05) and comprehensive group had better curative effect than incision group. Conclusion Incision therapy is easy, safe and effective on chronic throat impediment, and it will have better curative effect when combining with other proper external therapies of Chinese medicine.

摘要:The cultivation of clinical thinking ability is a key point for Chinese medical students, however, the weakness of clinical thinking ability has become a prominent problem even in the postgraduates. There are quite a lot problems appeared in the case report writing by postgraduate, while reinforcing the training in case history writing is an important method to improve this ability. The training might take commonly-seen diseases as breakthrough points, and is divided into 3 stages. Stage one includes writing case history independently and finding out the problems, and emphasis is put on the content of history, using standardized terms and language logic. Stage two includes forming a frame after repeated discussion, and selecting commonly-seen diseases accorded with acupuncture-moxibustion features, such as facial paralysis and headache for compiling a framework for case history writing. Stage three includes intensifying the training by application of the frame. The frame may guide the students to think of clinical problems step by step, correct the missing and mistakes in four examinations, prompt the key points in diagnosis and treatment, therapeutic principles as well as acupoint selection, and finally form the complete understanding to diagnosis and treatment of a disease. Both teachers and students in acupuncture specialty should be fully convinced of the importance of writing training of case history for improving clinical thinking ability.
