摘要:ObjectiveTo observe the effects of various electric stone needle intervention programs based on factors such as rotation speed, time, and vibration on the body’s skin infrared thermal imaging and blood flow, and to explore the best solution for electric stone needling.MethodsA three-factor two-level orthogonal table L8 (27) was designed, and 80 healthy subjects were randomly assigned to 8 groups, each with informed consent. After the subjects entered the laboratory, they sat quietly for 5 minutes to adapt to the indoor environment and keep the indoor temperature constant.Neiguan(PC6) and Ximen(PC4) points on the route of the pericardium meridian of Hand-jueyin in the left forearm of the subject were marked. An instrument was used to detect the infrared thermal image and skin blood flow of the area between Neiguan(PC6) and Ximen(PC4) points before the intervention of the subjects, and the results were recorded. The operator held the Bianstone drill and moved evenly along the pericardium meridian in the direction and part of the left forearm from Quze(PC3) to Daling(PC7). During the operation, a moderate pressure perpendicular to the skin was applied, and the treatment plan of each group was strictly followed (different rotationspeed, time and vibration). The infrared thermal image and skin blood flow of the area between Neiguan(PC6) and Ximen(PC4) were immediately detected and recorded after the subject’s intervention. And the data were recorded 5 minutes, 10 minutes, and 15 minutes later. Finally, statistical processing was performed.Results① There were significant differences between groups immediately after intervention and before intervention. All groups of infrared thermal imaging had significant statistical significances (P<0.05). Except for the 5th and 6th groups, the blood flow changes immediately after the intervention were compared with those before the intervention, which were all statistically significant (P<0.05). ② Each group showed no significant after-effects after the intervention, as compared with those before the intervention. Infrared thermal imaging and blood flow changes at 5 minutes, 10 minutes, and 15 minutes after the intervention were not statistically significant, as compared with those before the intervention (P>0.05).③ Through the analysis of the orthogonal experiment, it could be seen that the degree of influence of various factors on infrared thermal imaging was factor B(time)>factor A(rotation speed)>factor C(vibration), and the degree of influence on blood flow was factor A(rotation speed)>factor B(time)>factor C(vibration). The results of analysis of variance showed that factor A (rotation speed) and factor B (time) were significant factors affecting infrared thermal imaging and blood flow, with statistical differences (P<0.05), among which the infrared thermal imaging results showed two levels of factor A(rotation speed)(factor B(time), with statistical differences (P<0.05).Conclusions① Electric stone needling has a significant immediate effect on the body’s infrared thermal imaging and skin blood flow; ② Factor A (rotation speed) and factor B (time) are significant factors affecting the body temperature and skin blood flow; ③ The best tone needle programs that affect body temperature and skin blood flow are high speed (1800 rpm), long time (lasting 2 minutes), and small vibration (shock amplitude is about 0.3 cm).

摘要:ObjectiveTo explore the efficacy of the comprehensive rehabilitation program of traditional Chinese medicine(TCM) in improving spastic state of ischemic stroke hemiplegia with modern rehabilitation techniques as controls and with different degrees of neurological deficit as a hierarchical basis.MethodsA total of 147 cases with spastic hemiplegia of ischemic stroke were included and randomly divided into the experimental group and the control group according to the random comparison table with a ratio of 2∶1. The TCM comprehensive rehabilitation program and modern rehabilitation techniques were applied respectively, and the 2 groups were both given basic treatment. Evaluations were performed on Day 1 and Day 21 by using the Modified Ashworth Scale (MAS) and the National Institute of Health Stroke Scale (NIHSS). The severity of the disease was stratified and the curative effects were compared.ResultsAfter 3 weeks of treatment, the MAS scores were both improved of the experimental group and the control group(P<0.05), with the effective rate higher in the experimental group than in the control group. After stratification, the MAS of the experimental group with severe disease was significantly improved compared with that before the treatment(P=0.019), and there was no statistical difference in the control group of the same stratification(P=0.092). However, there was no statistical difference between the experimental group and the control group(P=0.814).ConclusionsThe TCM comprehensive rehabilitation program can improve the spasm degree after ischemic stroke. For patients with worse neurological deficits, TCM comprehensive rehabilitation program shows a better improvement trend than modern rehabilitation techniques.

摘要:Doctors, nurses and patients and their families have a specific significant relationship due to diseases. The advances of diagnostics and treatment technology and the drive of interests have affected the relationship between doctors and patients. Since the beginning of this century, the introduction and development of narrative medicine have played an important role in remolding the humanistic spirit in medicine. Medicine is benevolence, which is the important connotation of medical humanities. The study of narrative medicine helps to implement the concept of benevolence and harmony. Clinical nursing work embodies benevolence, compassion and empathy, which are both scientific and artistic. Nurses will be a new force in the practice of narrative medicine. Narrative medicine not only focuses on the independence of individuals, but also builds a doctor-patient moral community of groups, emphasizing the harmony of medical patients and their families to jointly safeguard the beauty of life and health. Narrative medicine promotes the revival of medical humanistic spirit, emphasizes that benevolent people can love everyone, values human ethics, jumps out of the trap of material labor, and helps medical science to maximize the effect of medical behaviors and serve people’s well-being.
摘要:Four kinds of doctor-patient divergences have been put forward in narrative medicine, namely, recognition of death and of etiology, the context of disease and negative emotions such as shame, blame and fear. However, from the perspective of traditional Chinese medicine(TCM), the doctor-patient divergences, influenced by Chinese culture, shows its own characteristics. This study was aimed at investigating the influencing factors on the doctor-patient divergences with Chinese characteristics, in the following aspects: the homology of traditional Chinese culture between doctors and patients, humanity in diagnosis and treatment of TCM, psychosomatic and spatiotemporal characteristics of the TCM model, the extension of TCM view about life and death from the perspective of traditional Chinese culture, in order to shed light on the hidden advantages of TCM in bridging doctor-patient divergences, and providing reference for clinical practice.

摘要:ObjectiveTo observe the effects of neurotoxic Aβ42 on the permeability and Aβ transportation of blood-brain barrier(BBB) in vitro and the role of Qingkailing Injection.MethodsThe BBB in vitro was established by culturing cerebral microvasculature endothelial cells of Balb/c mice and then they were divided into the normal group, model group and Qingkailing group. The transendothelial electrical resistance (TEER) and the permeability to HRP were used as indicators of changes in BBB permeability. The concentration of Aβ40 and protein expression levels of RAGE, LRP-1, Claudin-5, Occludin, PKC and ERK were measured by enzyme-linked immuno sorbent assays and western blotting respectively.ResultsAs compared with control group, the TEER and the expressions of claudin-5 in BBB decreased significantly in the model group(P<0.05, 0.01), while the permeability to HRP, the concentration of Aβ40, the expression of RAGE and PKC increased (P<0.01). Compared with model group, Qingkailing effectively decreased the permeability of the BBB(P<0.05, 0.01), promoted the expression of claudin-5 (P<0.01) and down-regulated the expressions of RAGE and PKC (P<0.01).ConclusionsQingkailing Injection has a protective effect on the structure and function of BBB. It suppresses the permeability to Aβ40 through intracellular and extracellular transport pathways respectively, by down-regulating the expression of RAGE and up-regulating the expression of claudin-5, breaking the vicious cycle of AD. This beneficial effect may be linked to the inhibition of the PKC signal pathways.
摘要:With the "New Theory of Qi and Blood" published in "Modern Clinical Chinese Medicine" , the importance of Qi and blood has attracted more and more attention in academic circles, but the status of Qi and blood in traditional Chinese medicine has not been fully explained and further research is needed. Indicators are commonly used methods for measuring and observing targets, and they are the angle of observation of health and disease in modern medicine. There has been no clear index concept in traditional Chinese medicine previously. This article holds that qi and blood are the core of the core theory of traditional Chinese medicine, and are the main observation indicators and main grasping methods of traditional Chinese medicine. This article attempts to explain this from the following four aspects. Firstly, the relationship between Qi, blood and the core theories of traditional Chinese medicine such as yin and yang, the five elements, visceral manifestation, meridian, etc. is analyzed.Secondly, the status of Qi and blood in the four diagnosis methods of observation, hearing, inquiry, and pulse-taking is explained. In addition, Qi and blood from a clinical perspective as the main focus of TCM clinical practice are analyzed. Qi and blood are scattered in various organs and parts, and are composed of a variety of small indicators. TCM practitioners use different senses to collect them to form indicator groups. The modernization of traditional Chinese medicine can not only be satisfied with the description of the body’s Qi and blood state, but also needs to use modern technologies to accurately quantify Qi and blood to better serve the scientific research and clinical practice of traditional Chinese medicine.

摘要:In modern traditional Chinese medicine, pathogenic dampness is regarded as one of the six evils transformed by the six qi of the tian (heaven). It is endowed with the attribute of " the tendency of dampness is downward and easy to invade the Yin position" .This expression comes from Su Wen· Tai-yin Yang-ming Theory," For those who are hurt by dampness, their lower part of the body suffers first" . Inspired by statements in other chapters in Huangdi Neijing,we reanalyzed this sentence, and found that the dampness here referred to the dampness from the earth, different from that of the six evils, in terms of concept, nature and pathogenic characteristics. The dampness of the ground easily hurt the skin and muscles, leading to muscle joint pains, amyotrophy, and cold limbs. In terms of treatment, it can be treated by following the principles of strengthening yang and dispelling dampness from Synopsis of the Golden Chambersto with method of inducing sweating and diuresis, with Atractylodis as the main medicine for its treatment.

摘要:Interstitial cystitis refers to chronic non-bacterial cystitis caused by unknown factors. It is one of hard diseases in urology. The condition is long and recurrent, which seriously affects the patient’s physical and mental health and quality of life. Interstitial cystitis is located in the bladder, and is closely related to the liver, spleen, and kidneys. It is mostly based on the deficiency of the spleen and kidney as its root cause, and the dampness-heat of the bladder as its symptoms, often accompanied by liver depression, with blood stasis as its pathological product. This disease is a mixture of deficiency and excess, and in treatment, the priority of the deficiency and excess of the conditionand the priority of the syndrome should be distinguished during treatment. At the same time, the mucosal lesions of interstitial cystitis under cystoscopy, can be included in the category of " internal ulcer" , and treated with traditional Chinese medicine, both internally and externally. The combination of " oral decoction" and " external perfusion" can improve the clinical efficacy of interstitial cystitis.

摘要:Cheng Guopeng and Fei Boxiong were both famous TCM practitioners in the Qing Dynasty. They both had their own characteristics in the treatment of consumptive diseases. Cheng Guopeng held that blood was more important when compared with Qi, and that Yin was more important when compared with Yang; as for the treatment of consumptive diseases, the first step is to arrest coughing, and equal attention is given to the treatment methods of clearing and enriching, and warming and tonifying. Moreover, for the damage of five zang organs, the kidney, foundation of prenatal existence, should be supplemented; the treatment of consumptive diseases is focused on health preservation and regulation. Fei Boxiong held that the impairments of consumptive diseases should be divided into Yin and Yang; exertions of the five zang organs should not be attributed only to the kidney; the impairments of the seven emotions should be eventually attributed to the heart; the treatment of consumptive diseases should be focused on the spleen and kidney. Both of the two practitioners paid attention to dietary care and mental regulation in the treatment of consumptive diseases, and had similarities and differences in warming and tonifying the spleen and kidney. This comparative analysis is helpful to further understand their academic thoughts, which is quite enlightening for the clinical treatment of chronic consumptive diseases.

The effect was better with 7 doses (74.62%).The post-medication stools increased, accounting for 50.85%. A total of 166 kinds of medicinals were used to modify the formula.Six main diseases and 11 main symptoms were screened out.ConclusionsThe Taohe Chengqi Decoction pattern could occur in both men and women, more in women. The majority of patients are young adults.Constipations, spasmodic pain in the lower abdomen, vexations, difficulty in micturition, abdominal distension, thirst, unconsciousness, fever, back pains, poor appetite and insomnia are the main symptoms. The scope of Taohe Chengqi Decoction used by modern TCM practitioners has been expanded continuously. It is in line with the pathogenesis of blood stasis and heat in clinical practice, which can be flexibly added and subtracted.
摘要:ObjectiveTo explore and analyze the medication law of traditional Chinese medicine in the treatment of EGFR-TKIs associated diarrhea (epidermal growth factor receptor, EGFR; tyrosine kinase inhibitors, TKIs) in non-small cell lung cancer.MethodsRelated literature was retrieved from CNKI, Wan Fang(WF) and VIP databases. TCM syndromes, TCM frequency, four qi, five flavors, tropism, etc. were counted, compatibility rules were analyzed and cluster analysis with ancient and modern medical case cloud platform V2.2.3. was conducted.ResultsThe highest frequency of TCM syndromes was deficiency of qi and Yin. Astragalus membranaceus was most frequently used, followed by atractylodes and Poria cocos. The main medicinals were medicinals that had a neutral nature, a sweet taste and those which were attributive to the spleen, stomach and lung meridians. Six drug combinations with high frequency and reliability were obtained, and four groups of medicinals in two categories were obtained, all of which had the main effects of enhancing qi and nourishing Yin, strengthening spleen and resolving phlegm.ConclusionsDiarrhea associated with EGFR-TKIS of non-small cell lung cancer is characterized by the deficiency of qi and Yin and the accumulation of phlegm in the spleen, and the common TCM syndromes are the deficiency of qi and Yin. The treatment focuses on promoting qi and nourishing Yin, strengthening the spleen and resolving phlegm, and the commonly used medicinals are medicinals with a neutral nature, a sweet taste and attributive to the spleen, stomach and lung meridians.

摘要:Acute respiratory distress syndrome (ARDS) is an acute clinical syndrome caused by various internal and external pathogenic factors. Its clinical manifestations are progressive dyspnea and intractable hypoxemia, and severe respiratory failure may occur in the late stages of the disease. Treatment from "state" is one of Professor Jiang Liangduo’s most prominent academic thoughts. Professor Jiang believes, in the thought of treatment from "state" , these three aspects should be mastered : evils invade the lung, and lung impairment causes Qi not to move body fluid, causing the formation of dampness and phlegm, which are its core pathogenesis. Therefore, tonifying Qi and diffusing the lung, clearing away turbid discharge run through the whole treatment; The patient’s state at the time helps to identify the etiology, location, nature of ARDS and strength between the body’s anti-pathogenic factors and pathogenic factors, which is the current pathogenesis. Methods of clearing away heat, resolving phlegm and promoting blood circulation can be used for symptomatic treatment. The underlying pathogenesis, namely ARDS, continues to evolve into the condition of heat entering the pericardium, Yin exhausting and Yang collapse. Then it is necessary to awaken the mind and resuscitate, restore Yang and the adversity. Only by mastering the above contents can we have a more accurate and comprehensive understanding of ARDS and be more confident in diagnosis and treatment.

摘要:With regard to treatment of heart disease by ascending and lifting the Qi, there are few literatures available. Professor Liu Yujie explored the ancient and modern literature of " great Qi" and " ascending and elevating the Qi" , and found that the heart diseases were mainly caused by the deficiency and impairment of great Qi, and thus proposed a new idea that ascending and elevating the great Qi should be the first priority in the treatment. In terms of treatment, the prescription is based on ShengxianDecoction. In this paper, Professor Liu Yujie’s experience in treating four kinds of heart diseases, such as bradyarrhythmia, is described in detail, and verified with clinical medical records.

摘要:Chordoma is a rare subtype of bone cancer. Wide resection with negative surgical margins and radiotherapy are the main effective treatment methods. Although there is no preferred regimen for chordoma, the targeted therapy can still be recommended. On the basis of traditional Chinese medicine (TCM) syndrome differentiation and treatment, Professor Yang Yufei combines TCM treatment with the low-dose targeted therapy to achieve PFS for 12 months in a chordoma case. This article may help inherit valuable experience of TCM experts and take advantage of TCM in the clinical practice for chordoma.
